As it takes several weeks for your records to arrive, you are asked to complete a Health Questionnaire as part of the registration process, giving us essential details of your health needs. 

We also like new patients over the age of 5 to have a new patient check with the practice health care assistant. This is a simple, often valuable check which can also serve as a useful introduction to the practice.

Patients registering under the age of 5 will also need a Health Visitor form which is available to fill in at the surgery.
